Graceful Transitions Senior Placement was created to help seniors and their families navigate one of life’s most important transitions with confidence, compassion, and personalized support.
Owner Linsey Sheffield brings more than 15 years of healthcare experience, including 10 years in acute care settings and 4 years in hospice care. Her background has given her firsthand experience supporting individuals and families through critical stages of aging, healthcare decisions, and senior care planning.
At Graceful Transitions, the goal is to help families understand their options, ask the right questions, and find senior living solutions that best meet their loved one’s unique needs while providing peace of mind every step of the way.

Linsey Sheffield is the owner of Graceful Transitions Senior Placement. With a heart for service and a deep understanding of the senior care journey, she is passionate about helping families find the right fit for their loved ones.
Her work is guided by the reminder in 1 Peter 4:10: “As each one has received a gift, use it to serve one another as good stewards of God’s varied grace.” For Linsey, Graceful Transitions is a way to use her experience, knowledge, and compassion to serve families during an important season of life.
As the dependent of a Naval officer, Linsey has lived in several parts of the country, including New Orleans, California, Corpus Christi, Virginia, and Georgia. This connection to military life has also given her valuable knowledge of VA benefits and resources available to veterans and their families.
Family is at the heart of everything Linsey does. She is a proud wife and mom to a wonderful 10-year-old daughter. Her family is Aggie through and through, and their home is shared with a 10-year-old Labrador Retriever and an energetic 2-year-old cat.
At Graceful Transitions, Linsey serves families with integrity, empathy, and expertise so they never have to walk through this process alone.
